Bonus Schedule
*Any qualifying perfect NEW order between $50 and $99 will earn 50 bonus points.
Any qualifying perfect NEW order, $100 and over will earn 100 bonus points.
A perfect order is one that is not delayed in our manufacturing process by questions due to copy problems, incomplete specifications, or other problems.

Please note that all orders for Medical Billing Forms (CMS) DO NOT qualify.

Sterling Rewards Membership Rules
(Revised 09/06)- Membership in Sterling Rewards will be granted by Sterling Business Forms, Inc. through enrollment as a Sterling Rewards Member.
- Beginning October 23, 2006, points for qualifying orders will be automatically deposited into your account once your qualifying order has shipped and billed.
- To obtain points for each qualifying order, you will need to note your Sterling Rewards account number on your Purchase Order at time of submission.
- Beginning October 23, 2006, each point is valid for 6 months from the date earned. Your Sterling Rewards account will contain only valid points collected within the immediately preceding 6 consecutive months. If points are not applied toward an Award within 6 months of the date earned, they will be deleted from your Sterling Rewards account balance.
- Sterling Business Forms, Inc. reserves the right to cancel the membership of any Member at any time, regardless of point balance, or account activity. If an account must be cancelled by Sterling Business Forms, Inc. all points in account will be considered null and void.
- Membership in the Sterling Rewards program is for individuals only. Individuals must enroll separately and may not pool or combine points with other Members. Only one rewards account number per purchase order. Members are limited to one account. Corporations and other entities may not be enrolled as Members.
- Only the person whose name appears in the Sterling Rewards account and who actually submits a qualifying Purchase Order will earn points.
- All Awards are valid for 12 months from the date of issue. Sterling Business Forms, Inc. is not responsible for and will not replace lost, stolen, or destroyed Awards.
- Sterling Rewards Awards are non-transferable. All transactions in the Sterling Rewards program are subject to review and adjustment by Sterling Business Forms, Inc. Sterling Business Forms, Inc. will cancel your account and Awards if misuse or violation of Sterling Rewards rules is detected.
- Address or name change requests may be submitted in writing to Sterling Rewards, P.O. Box 2486, White City, Oregon 97503. The written request must contain your signature and account number. Changes may also be initiated on our web site.
- Sterling Rewards Award cards are not credit cards.
- All taxes and/or user fees that may be imposed on Awards are the responsibility of the recipient.
- If you have any questions regarding this program, you may call 1-800-759-3676.
- Awards will be issued within 30 days of point redemption request.
- All orders will ship on regular delivery schedule.
- Only employees of dealer accounts in good standing will be eligible for the Sterling Rewards Program. Delinquent accounts are subject to suspension at any time.
