CMS Sterling Business Forms, Inc. carries the most current government revisions of CMS-1500 and UB-92 Health Claim Forms. For your convenience, we also offer Dental Claim Forms and Envelopes to address your customer's needs.

We are now pleased to announce that we have the NEW CMS-1500 (Rev. 08/05) claim form. As of October 1st 2006, providers can use either the current (12/90) version or the revised (08/05) version of the 1500 Claim Form as their billing needs and software requirements dictate. On April 1st 2007, transition to the new form will be mandatory for all providers.

We recommend creating a Postscript file and then processing it with Acrobat Distiller. Download our distiller settings file or use the following settings:
  • Compatibility version 4, 5, or 6 (PDF 1.3, 1.4, or 1.5).
  • Press optimized.
  • Resolution 2400 dpi.
  • Embed all fonts.
  • Leave color unchanged.
  • Maximum quality.
  • All compression off.
Prescription Pads California Senate Bill 151 requires that Level II - Level V prescriptions be written on forms that are compliant. Don't risk having prescriptions unfilled by your patient's pharmacy or assume the liability of not complying with state law. We can help make sure your office meets the new requirements.
